Isolation Notes

This new temporary system will enable people who are advised to self-isolate to obtain an isolation note via NHS 111 online or the NHS website. This can then be used as evidence for absence from work.

What is an isolation note used for?

  • It is for employees who need evidence to cover a period of absence due to coronavirus. Employees can self-certify for 7 days, but beyond that, if they have been advised to self-isolate or share a household with someone who is symptomatic and self-isolating, they may be asked by their employer to provide evidence. The isolation note is evidence to be given to an employer.

When will the isolation note be available?

  • This new service will be available from Friday 20 March

Beaconsfield Closure

From Monday 30th March Beaconsfield Primary Care Centre will be closed between 12.30pm-1.30pm.

This includes both the doors and phone lines.

The answer machine on the telephone will give you information on what to do in an emergency.

Coronavirus Information

We have received specific Instructions from our CCG to ask you NOT TO ATTEND the surgery. This is to reduce the risk of Coronavirus spread to you, your fellow patients and Health Care Workers.

We are now only able to deal with urgent medical matters. If you feel that your medical problem is urgent, you can still ring the surgery, but you will be offered a Telephone Consultation rather than a face to face appointment.

Alternatively, please use our E-Consult Service. For patients who already have pre-booked appointments, we will be in touch with you directly by text or phone call.

Online Appointment Booking

The surgery has been advised from NHS England to suspend all online bookings until further notice as a means of mitigating Coronavirus.

We apologise for any inconvenience.

Prescription Requests

Please allow 5 working days from the day you request your prescription at the surgery to the day you collect from your pharmacy due to the increase in volume of requests at this time.

If you do not have a nominated pharmacy for electronic prescriptions we strongly advise you to contact us with the pharmacy of your choice in order for us to electronically send your prescription to the pharmacy to mitigate any risks.

We will not be issuing any prescriptions earlier than the 7 days allowed and we cannot issue extra just due to the coronavirus.

High Risk Patients

The Government has issued new guidance for people who are at very high risk of severe illness from coronavirus (COVID-19) because of an underlying health condition. If you're at high risk, you will be contacted by the NHS by Sunday 29 March and strongly advised to stay at home at all times and avoid any face-to-face contact for a period of at least 12 weeks from the day you receive your letter.
